help with submissions

I'm trying to submit a double page layout. The requirements are a double page spread plus a close up of the specifics. the problem is there is only one browse box for uploading and no (apparent) way to submit page two or to leave comments about the technique (also requested).
Can anyone out there help me to figure this out.
